Six Steps to success to providing end of life care in care homes

Tameside and Glossop Integrated Care NHS Foundation Trust is holding the Six Steps cohort two course from February 2018 to September 2018.  This is a course that has been recently reviewed and will provide care homes with the training and resources to provide and evidence excellent care and communication for their residents approaching the end of their lives.

We invite all care homes and nursing homes in Tameside and Glossop to enrol on to this course, which will require two care home staff per home (one to be manager, deputy or owner) to attend and participate in eight training sessions, with completion of a portfolio alongside these, and cascade this training to the rest of their colleagues. You will also receive support visits throughout the course from the facilitators.

This training package is recognised by the Quality and Funded Care teams as evidence of being able to provide good end of life care.

The course dates below are for cohort two, and need to be booked via Eventbrite. There is a small fee of £9 per person (£18 per home) per date to cover room booking costs and course materials. There is a small booking fee on top of the ticket price. This fee needs to be paid on booking to secure the place.  Last date for booking is seven days prior to the course date. Any dates with fewer than 10 places booked may be cancelled.

Venue: Stanley House Masonic Hall, Audenshaw, M34 5GB

Dates: (All dates are 9.30am – 2.30pm)

Please direct questions to the facilitators Julie Moran by email: Juliemariemoran@nhs.net or Fiona Horrocks by email: Fiona.Horrocks@nhs.net.
