Six Steps to success to providing end of life care in care homes

We are looking across all care homes for expressions of interest for our 2019 Six Steps cohort.

The revised Six Steps to Success 2019 programme aims to enhance end of life care through facilitating change, supporting, empowering and educating staff to develop their roles around the provision of end of life care within the home care setting. 
This 2019 cohort will be for Managers, Deputies and senior leads solely, it will run over a period of nine months, this will include a monthly session which encompasses education around the following areas Organisational Change, Communication Skills, End of Life Care, and Advanced Care Planning. The programme will support collaborative, multi-professional working across all settings and, following successful completion of the programme, the care home representatives will be encouraged to continue their professional development to stay at the forefront of end of life care within their locality.
All we ask is that you commit to the programme through attendance, then by supporting, embedding and sustaining the required organisational change, which will be demonstrated in a completed portfolio of evidence.
